Invitation and consent
You are invited to the study's name. Press Yes, I am interested or No, thank you. Reply by the date when the page gives one.
The page says: "Saying yes does not enrol you and is not consent." A yes is not consent yet. The study team contacts you next. A no stops reminders about this invitation. You may Send note, Stop emails about this study, or Send me the results when published. You have already replied means write to the study team to change it.
Questions first
When the study asks screening questions, they come before the form, under A few questions first. Each one is Yes or No. A question marked optional can be left blank. Press Continue to the consent form.
If an answer needs a closer look, the heading is The study team is reviewing your answers, and the form does not open yet. If you do not meet the study's criteria, the heading is Thank you for your answers, and the form stays closed. Write to the study team if you think an answer was wrong.
The consent form
The form is the published wording. Nothing is added inside it. Print or save a copy sits outside it.
Tick I have read this form, I understand it, and I agree to take part. I agree stays unavailable until you tick it: "Tick the box above to agree." The page then says Your consent is recorded. If agreeing also enrolls you, it says you are enrolled in this study. Otherwise the study team tells you what happens next.
I do not agree records that you chose not to take part. The page says: "If you change your mind, this link still works." Press Review the form again to agree later. If you already said you do not agree, you can still agree below. The same three optional buttons follow a no here.
When the study offers it, an optional tick reads With my permission, acknowledge me by name in the resulting publication. Leave it unticked and the page says the publication will not name you.
After you have agreed, changing your mind is leaving, not I do not agree again. If the form is not published, the heading is The consent form is not ready, and nothing is recorded.
